MODIFIED Feedback Form in Downloads |

 ArchangelArtifacts writes "Ok I found the original PHP code found in the Feedback Form.

The above script is line per line of the Feedback Form, except it is now coded for PHP-Nuke.
Its funny how alot of the times the PHP coders are forgotten in credits when someone codes it for PHP-NUke.

I dont know who wrote the original script, in the feedback form index.php it says it is "Based on php Addon Feedback 1.0 Copyright (c) 2001 by Jack Kozbial". I'm so confused. (Easily done though) ;-)
Hey the code in several of these scripts look familar...Hmmm.
Anyways.....
I modified the Feedback Form into a simple Contact Us Module with tables, $bgcolor1, tabled error messages in red and added the thanks.gif from the script found at http://www.diction.net/download/.
Open up the language file and change the define _CONTACTUSNOTE to your own information ie. address and phone number.
Drop in the modules folder, activate and go....
It is a very simple script, I mean if someone puts "a" for their name, "a" for their email and "a" for thier message it sends it.
Please do not do this at my site. ;-)
Go ahead and check out the errors messages by just hitting send with no info in.
Drop a line to say Hey, to see the other changes.
Oh Yeah I added in IP address of the sender at the thank you page and in the email.

(User Info | Send a Message) http://www.nukebbmods.net | there is a similar Contact module from NukeStyles, except it does error checking/field checking, and allows the user to contact a pre-defined list of departments (defined by the admin in the administration panel)
the NS Contact form also grabs the user's info for the email, which is nice so that you have an idea of who is mailing you |

I wasnt trying to make a new contact module, Shawn did a great job with his mod, I just redid the one that originally comes with PHP-Nuke. As it is I have 42 modules through the admin. I didnt need anymore. I just redid this for my own site from the original script, added the thanks.gif from the ORIGINAL PHP code and made it more like the original code. http://www.diction.net/download/ has the original code from 1997, line per line. If I add error checking and throw in an admin interface, then it would have been a New Contact Us Mod and not a Modified Feedback Form. If you can use it, cool, if not, then dont.
