Security graphics broken after applying secfix 4 on nuke6.7

I was running phpnuke6.7. Login is perfectly fine. After I applying security fix4 by replacing files and lines as instructed in fixchanges.txt.

What could be wrong? Fortunately, I backed up all the files replaced. Anyone knows which file is crucial to the appearance of this problem?

The files included with the sec-fix patches are meant for Nuke 6.5, users of newer versions must apply the fixes manually
