Inserting Html code? #include virtual="/Assets/form.htm

Im very new to Nuke. I want the below to be on my Messages Administration.

Or better yet, in my Theme!!!

I am unsure how to have the below show up in my Messages Administration page.

Its just a simple search engine for google and yahoo.

See it working at http://www.12string.net/Assets/form.html

I looked at several posts. The explaniations were way to complex
I don't think they pertained to my need.

Below is the URL and the #include virtual="/Assets/form.html"
that I have been inserting with NO LUCK in my Messages Administration.

#include virtual="/cgi-bin/searchem.cgi

or
#include virtual="/Assets/form.html"

The code does work with my html pages.
