MEMBERS LIST ERROR Could not query users

So you know what configuration I have, nuke 7.9 w/ 3.3 patch, latest phpBB

The problem is with my members list. It was working fine with old version of phpBB, just updated it to the latest version because I am know planning on having a forum on my site. when I go to the members list it shows this error

Looks like you are using the Add Fields mod, part of the Approve Membership mod. You'll need to either remove the mod or reinstall the mod.

I have reinstalled it 2 time now with no luck, Just wanting to know if any one has any thoughts as to what is causing it or how to fix it. Posted on the AM site for help with no luck their, so I thought maybe some one here would have the knowledge that I do not to shoot me in the right direction

I never tried them, but it seems you made some mistake, cause your files are asking for db values that do not exist in your DB.

Install the mod exactly has described in txt guide.
