When trying to run nukesql.php I get the following error messages, any advice?

Well we were unable to resolve this issue in the new install forum, so I am hoping someone might be able to help me out here.
Does anyone have any thoughts?
the conntest.php works just fine.

I think what Raven was trying to say is if there is a thread already started do not start a new one with the same problem, if you can't get nukesql.php to run then use nuke.sql through phpMyAdmin.

I have no problem getting the db created using command lines if necessary, it's the two error lines that I'm getting that are a problem. It seems to be looking for something in the include directory that is not there, but I am not a programmer and am not 100% sure what is going on.
