Another thing that might be causing the problem is IE.
Have you applied all the fixes by chatserv by any chance?

Sorry if this was covered previously, but what fixes are those?

It seems to happen on multiple browser versions (as well as on Macs and PCs).

Basiclly those patches are done by many people, mostly by chatserv, Raven and NSN team and then pack them up in a pack.

While I haven't changed much as far as function goes, I have made a lot of cosmetic tweaks, and I really don't want to lose them. Are there any specific files in the 7.4 patch that might help (anything in includes or modules/Forums specifically)?

I don't think it does effects on your themes, just the code. If you have made some code changes in phpBB module then you'll have to add them again.

I'll have to go through the files and see what I've changed. I'll try and update the includes and Forum module files tonight.

Also, this could be related to includes/sessions.php.
