Hey guys,

Im using formgen atm and I used Fantastico Via Cpanel to install it. It worked fine up until a few days ago when i stopped recieving the forms that where submitted. I have several other forms on the website as well and im having problems recieving them as well ;-(. Anyways Im trying to find out how to make a form just like this one that can be published on the php page rather then be uploaded as a HTML file. Heres whatI want my form to look like:

That is probably done in html but set as a module on the site so it looks integrated. All you have to do is code it in basic html to make it look similar...then create a module and call the html file from the modules index.php. If you create the html file I can show you how to call it from a module. Or if you PM me I can set you up with a basic enlistment form scipt and you can edit how you like.
